maggot treatment for wound debridement

excited nak share 1 pengalaman. skrg aiman tgh posting ortho n tgk bnyk diabetic foot yg memerlukan wound debridement (WD). (layman term: buang tisu mati) tapi skrg bkn nak cerita pasal diabetic foot, tp tntg nenek yg dimasukkan ke hospital sbb ada infected bed sore.

nak dijadikan cerita, dr plan utk buat wound debridement pd nenek tp seperti biasa prosedurnya kne screen la dulu fit ke tak utk diberi general anaesthesia (bius sampai tak sedar) tp keputusan echocardiogram (utk cek jantung) disahkan ada masalah sikit dan tak dapat proceed pada WD.

alkisahnya, mak sedara aiman yg juga seorg matron di salah sebuah hospital government mencadangkan kpd dr utk guna maggot treatment. dan aiman of coz la terkebil sbb tak pernah dgr pon treatment tu. ingatkan tu rawatan alternatif tapi rupa2nya da banyak government hospital guna treatment ni. dan maggot treatment ni jgk telah dibuat kajian oleh IMR.

aiman tgk sndiri sterile larvae yg didatangkn khas dari kl ni, dgn harga rm1 per ekor. dalam 1 vile tu ada 200, n dr guna 2 vile utk nenek. antara mode of action larva ni adalah:

* debride wounds by dissolving only necrotic, infected tissue;
* disinfect the wound by killing bacteria; and
* stimulate wound healing.
maggot ni very selective, x makan tisu2 badan yg sehat. ambk yg da mati je. so yg sehat dia tak kacau. kira baik la kan.. da ada granulation tissue senang la wound tu nak sembuh.

dan jugak maggot ni cost effective:
  • A typical chronic wound costs RM2225 to debride in the Malaysia today.
  • It takes an average of 89 days to achieve with conventional dressings.
  • Sterilarvae products cost RM200 to debride the same wounds

Advantages and disadvantages of maggot debridement therapy (“MDT")

  • efficacy, as demonstrated in several small but significant controlled clinical studies.
  • Takes about 15-30 minutes to apply a secure dressing to keep the maggots in place.

  • Maggots are highly perishable and should be used within 24 hours of arrival.
  • Simple enough that non-surgeons can use it to provide thorough debridement when surgery is not available or is not the optimal choice. This means that it is also possible to provide surgical quality debridement as an outpatient or in the home.
  • Low cost of treatment.

n lagi satu yg sy belajar. ada satu alat nama air ring. alat ni mak saudara sy cari sbb dr kata tmpt wound tu xleh kena pressure. so nak position snang la sket.. cari punya cari kat farmasi x jumpa. akhirnya kami pn beli la pelampung utk swimming. hehe.. hidup mesti kreatif. sy pernah tgk air ring tu sbb customer yg sy antar ke airport duk atas air ring sebagai physiotherapy utk coccyx fracture (patah tulang punggung)

as a conclusion, life is a learning process. tanpa kita sedar, kita mempelajari sesuatu yg baru dari org sekeliling kita.
